IMS Scenario

The Incident Management System provides the tools you need to follow up on an incident and help make sure that your organization prevents similar incidents.

Consider the scenario below and review how IMS can help you manage it smoothly and efficiently.

Incident Managing the Incident in IMS

1. Joe, one of your employees, sustains an arm injury due to a fall while he is at work.

Several other employees observe the fall. They notice that Joe fell because a piece of equipment was leaking grease and there was grease on the floor.

1. Employees log in to PureSafety and submit their incidents about Joe’s fall, directly from their PureSafety home page.

2. The Safety supervisor reviews these incidents.

2. All the incidents display on the News Feed screen in IMS, where the Safety supervisor can review them as soon as they are submitted.

3. The Safety supervisor notes that all the incidents have to do with the same main event – the piece of equipment that was leaking grease on the floor.

3. The Safety supervisor creates an event record about the spill and links all the incidents he has received to the event. He also links two incidents about this equipment that were made before this incident occurred. Later, he can also add incidents and notes from follow-up checks in the event record.

4. The Safety supervisor begins to follow up on this incident.

For example, he might ask Joe to describe the incident, request that Joe’s manager document the injury, assign the responsibility to clean up the grease and fix the source of the leak to the facility maintenance staff, and ask the floor supervisor to check if there are any other spills on the floor related to similar equipment.

4. The Safety supervisor creates specific tasks and assigns them to other employees.

5. Joe, his manager, maintenance staff, and the floor supervisor receive their assignments, take care of them right away, and let the Safety supervisor know that they have completed them.

5. These employees see their newly assigned tasks on their home page in PureSafety. Some users can complete their tasks directly in IMS, for example, if they need to fill out a form or create an injury record. Other users have to complete their tasks outside the system and then go in to IMS to record that they are done.

6. The Safety supervisor conducts an investigation of the spill.

For example, he may review maintenance records to see the last time maintenance staff checked and cleaned the equipment, he may interview the employees to get additional details, and he may take pictures of the equipment. Once he identifies the root cause, he can communicate this issue with other facilities within the organization that have the same equipment and ensure they take preventative action to mitigate such leaks.

6. The Safety supervisor records all his investigation in the event record in IMS. Other members of the Safety and management teams can see his notes and resolution and they can refer back to this information as necessary. Managers at other facilities can see a description of the root cause and recommended preventative actions and implement these actions at their facilities.

7. If necessary, other members of the Safety or management team are required to verify that all the appropriate follow-up tasks, investigation, and corrective actions have been completed.

7. IMS users who are responsible for verifying that tasks are completed correctly can see these tasks on their home page once the tasks are completed and ready for their approval. They can review these tasks and verify that they have been completed as expected.

8. Once all the follow-up tasks are completed, the Safety supervisor reviews all your data about the fall, employees’ incidents, and follow-up tasks. After he makes sure he did not leave anything out, he can record that this event has been taken care of properly and that he has made sure a similar event will not occur again.

8. The Safety supervisor reviews the event he created, confirms that all the follow-up tasks were completed correctly, and closes the event.
