This report provides the OSHA's Form 300A, Summary of Work-Related Injuries and Illnesses (Rev. 01/2004).

You can run this report from the Reports Directory or from the Injuries screen.

OSHA 300A Form

Running the report for OSHA's electronic submission requirements

Use the CSV option to export the report data into a format that can be used for electronic submission required by OSHA.

After exporting the report, you must open the file, replace the value in the Size field, and delete the NOTE (the top two rows of the file).

Replace the value in the Size field with the value below based on the maximum number of employees who worked at the organization at any point during the year for which you are submitting the report:

3 More than 250

OSHA 300A form fields

The table below describes the OSHA 300A form fields and the corresponding PureSafety source fields. Many of the OSHA 300A form fields are populated with the sum of multiple fields from multiple injury records. The specific fields are listed in the source field column. Go to IMS > Injuries > Create or IMS > Injuries > Edit > Select an injury record to view individual source fields. The capitalized words in the PureSafety Source field column represent the tab on the Create Injury screen where the field displays.

(G) Total Number of Deaths

The sum of injuries that resulted in death and are recordable injury types.

(H) Total number of cases with days away from work

The sum of injuries that:

  • didn't result in death,
  • resulted in LWD, and
  • are recordable injury types

(I) Total number of cases with Job transfer or restriction

The sum of injuries that:

  • didn't result in death,
  • resulted in LWD that is less than 1 day,
  • resulted in RWD, and
  • are recordable injury types

(J) Total number of Other recordable cases

The sum of injuries that:

  • didn't result in death,
  • resulted in LWD that is less than 1,
  • resulted in RWD that is less than 1, and
  • are recordable injury types

(K) Total Numbers of Days away from work

The sum of all LWD.

(L) Total number of days of job transfer or restriction

The sum of all RWD.

The sum of injuries that are recordable injury types and are a type of injury.

(2) Skin Disorders

The sum of injuries that are recordable injury types with an injury type of Skin Disorders

(3) Respiratory Conditions

The sum of injuries that are recordable injury types with an injury type of Respiratory Conditions.

(4) Poisonings

The sum of injuries that are recordable injury types with an injury type of Poisonings.

(5) Hearing Loss

The sum of injuries that are recordable injury types with an injury type of Hearing Loss.

(6) All Other Illness

The sum of injuries that are recordable injury types with an injury type of All Other Illness.
