Observation with no Losses

Scenario

Joe, a worker, steps out of the production supervisor’s office. He has been given a rush job and is in a hurry. He throws open the door, almost hitting two employees and steps out in front of a forklift. The forklift driver manages to stop just inches from Joe. No one got hit by the door, and Joe narrowly missed being hit by the forklift.

The production supervisor, Dana, realizes she just observed a near miss with serious potential and files a near miss report, following her company’s incident reporting procedures. Next, Dana investigates the area where Joe almost got hit. She notices that there was a pallet with materials right outside her door that obstructed the forklift driver’s view so that he couldn’t see Joe when he came out of her office. She decides the pallet should be moved and that area should be kept clear to ensure drivers have good visibility and a similar incident does not occur.

Dana wants to record what she just saw.
  1. In PureSafety, Dana goes to the News Feed screen, and creates an incident about what she just saw.
  2. On the Description tab in the incident, she types what happened: “I saw Joe using a ladder rather than a picker to retrieve a heavy box from fourth shelf on Aisle A. He could have hurt himself.”
  3. Dana saves the incident, which displays in the News Feed.
Next, Dana wants to ask Joe to take his training immediately.
  1. On the News Feed screen, she selects the incident she just entered and clicks the Assign Task button.
  2. She types a title, selects Assign Training in the Type field, and selects to assign training directly.
  3. She also checks the Corrective Action box because this training corrects an issue that could eventually lead to an incident.
  4. She provides information about the assignment. When she is done, the task displays in the incident record on the News Feed screen. In addition, the training assignment automatically displays in the Assignments section on Joe’s home page. He can quickly see what he needs to do.

Joe logs in to PureSafety and sees his new training assignment on his home page. He watches it right away. When he is done, the system records that he has completed his assigned training.

A couple of days later, Dana wants to verify that Joe completed his training.
  1. Dana goes to the Assignments screen.
  2. She enters Joe’s name in the Assigned To field to filter the list and see only Joe’s assignments.
  3. She selects the training assignment she gave Joe a couple of days earlier and opens it.
Dana sees that Joe has completed his training. As a result, she wants to close the incident about Joe trying to lift the box incorrectly.
  1. Dana goes to the News Feed screen and edits the incident she entered about Joe a couple of days earlier.
  2. In the Status field, she selects Resolved and clicks Save.
