Near Miss

Joe, a worker, steps out of the production supervisor’s office. He has been given a rush job and is in a hurry. He throws open the door, almost hitting two employees and steps out in front of a forklift. The forklift driver manages to stop just inches from Joe. No one got hit by the door, and Joe narrowly missed being hit by the forklift. The production supervisor, Dana, realizes she just observed a near miss with serious potential and files a near miss report, following her company’s incident reporting procedures. Next, Dana investigates the area where Joe almost got hit. She notices that there was a pallet with materials right outside her door that obstructed the forklift driver’s view. The driver could not see Joe when he came out of her office. She decides the pallet should be moved and that area should be kept clear to ensure drivers have good visibility and a similar incident does not occur. |
