Near Miss

Scenario

Joe, a worker, steps out of the production supervisor’s office. He has been given a rush job and is in a hurry. He throws open the door, almost hitting two employees and steps out in front of a forklift. The forklift driver manages to stop just inches from Joe. No one got hit by the door, and Joe narrowly missed being hit by the forklift.

The production supervisor, Dana, realizes she just observed a near miss with serious potential and files a near miss report, following her company’s incident reporting procedures. Next, Dana investigates the area where Joe almost got hit. She notices that there was a pallet with materials right outside her door that obstructed the forklift driver’s view. The driver could not see Joe when he came out of her office. She decides the pallet should be moved and that area should be kept clear to ensure drivers have good visibility and a similar incident does not occur.

  1. In PureSafety, Dana goes to the News Feed screen. She creates an incident about what she just saw. The incident displays in the News Feed.
  2. Since she wants to investigate the near miss and implement some changes to prevent similar occurrences, Dana also creates an event from the incident.
Next, Dana investigates the near miss and types her findings.
  1. In the PureSafety, Dana goes to the Event Feed screen and opens the event she created about the near miss.
  2. On the Actions and Investigations screen, she creates a task, assigns it to herself, and types all her findings.
  3. On the Causal Analysis and Corrective Actions screen, she specifies that the pallet was a causal factor in the near miss. She creates a corrective action task, asking the maintenance staff to move the pallet and add a sign requiring employees to keep that area clear of any materials.
  4. She also decides it is important to remind employees that they have to be aware of their surroundings when they are on the production floor. She creates another corrective action task asking another floor supervisor to discuss this topic at the next staff meeting.
After the pallet has been removed and the staff meeting took place, Dana closes the event about the near miss.
  1. In the PureSafety, Dana goes to the Event Feed screen and opens the event she created about the near miss.
  2. On the Conclusion screen, she marks the event complete.
