Scenario

Joe, a worker, was standing beside his truck preparing to strap down a load of steel sprinkler pipe bundles when a forklift driver loading bundles on the left side of the truck bed inadvertently knocked a bundle off the truck bed. The bundle hit Joe, knocking him to the ground. Joe was transported to the hospital where he recovered. He was away from work for three weeks and then placed on restricted duty for six months.

In IMS
Joe’s co-workers record the incident.
  1. In PureSafety, they click Report Incident on their home pages.
  2. In the Loss Types section, they check the Injury box to indicate that this occurrence resulted in an injury.
  3. They type a description, such as “Joe was hurt when a bundle of pipes slid off his trailer.” or “Joe was injured this morning. We called 911 and he was taken to the hospital.”
  4. They click Submit.

Dana, the Safety supervisor, is reviewing the day’s incidents on the News Feed screen and reads the incidents about Joe’s injury. She edits the incidents and assigns them to David, who is the manager at Joe’s location.

David gets emails from the system. They notify him about the new incidents assigned to him. Since this incident resulted in an injury, David wants to investigate it and identify corrective actions. He creates an event in IMS to keep track of all these tasks.
  1. In the PureSafety, David goes to the News Feed tab and reviews each incident.
  2. He changes the status of each incident to In Progress.
  3. He selects one of them and clicks the Create Event button.
  4. Then, he links the other incidents to the event. He clicks the Link button in the function bar and selects To Existing Event. On the Select an Event window, he selects the event he just created and clicks Select.
David is also the OSHA Administrator at his location, so he creates an injury record for Joe.
  1. On the Event Feed screen, David opens the event he just created.
  2. On the Losses screen, he creates an injury record and provides information about Joe’s injury.
Next, David assigns a task to Gary, Joe’s immediate supervisor, to find out what caused the incident.
  1. In the event record, David goes to the Actions and Investigations screen and creates a task asking Gary to investigate the incident.
  2. When he is done, the task displays in the event record. On the event record, David can review its progress. The task also displays in the Assignments section on Gary’s home page, where he can quickly see what he needs to do.
Gary logs in to PureSafety and sees the new task David assigned him. Gary interviews witnesses and provides his findings in the system.
  1. In the Assignments section on the home page, Gary selects his assignment and clicks the Take Assignment button.
  2. He enters notes about what his investigation uncovered.
David sees Gary’s comments in PureSafety. He also talks to Joe, the injured employee, over the phone. Now, he is ready to add more details about the event.
  1. In the event record, David types a summary of the incident in the Description field on the Summary screen.
  2. On the Actions and Investigations screen, David types more details about the injury and attached file and pictures as needed.
  3. On the Causal Analysis and Corrective Actions screen, David creates a corrective action task asking forklift drivers to retake their safety training. He selects the Assign Training in the Type field. He selectsto assign the training directly. He types necessary information about the assignment.
  4. When he is done, the task displays in the event record. In addition, the training assignments display automatically in the Assignments section on the other employees’ home pages.
After all the appropriate follow-up tasks have been completed, David closes the event.
  1. In the event record, David goes to the Conclusion screen.
  2. In the Owners section, he clicks the Mark as Complete button beside his name.
