Create a Custom Investigation Form

There are several standard investigation forms available in IMS. However, you can also create custom forms by creating training units and then indicating they can be used in IMS investigations.

Note: The information captured in custom forms is NOT saved discreetly and is not available in any other fields in IMS. You must open the custom form attached in an incident or event in order to review it.

You can create three types of training that can be used in investigations.

  • Create a Rich-Text training unit to create document that displays information you type in, such as a questionnaire.
  • Create a File training unit to upload a file that users need to review. For example, you can upload a Word document with your incident review policies and procedures.
  • Create a Webiste training unit to add a link to an important Web site that employees should review, such as the Safety section on your Intranet.

Custom Form Properties

Follow the steps below to create these training units.

  1. Go to the PureSafety tab > Library > Create Training screen.
  2. Next, create the appropriate type of training.
    • Go to the Build Training section and click the Rich-Text option to create a rich-text training unit.
    • Go to the Reference External Resources section and click the File option to create file training unit.
    • Go to the Reference External Resources section and click the Website option to create a Website training unit.
  3. Provide all the information required.
  4. Click Create.
    The new training unit is created.
  5. Indicate that this new training unit should be available in IMS. On the Library tab, go to the New or Updated Training screen.
  6. Select the new training you created and click Open.
  7. Click Properties in the Actions menu.
  8. Check the Is Investigation box on the General tab.
  9. Click Save.
    Users can select this training as a custom form in investigation-type tasks.
